الوصف الوظيفي:

سيكون عامل/ة الحالة مسؤولًا عن تنفيذ إدارة الحالات الشاملة لحماية الأشخاص المعرضين لمخاطر مرتفعة. سيعمل تحت إشراف ضابط الحماية لضمان جودة وفعالية إدارة الحالات.

المسؤوليات:

إدارة الحالات الفردية:

• إجراء زيارات منزلية ومقابلات في المراكز لتقييم احتياجات العملاء والأسر.
• العمل مع العملاء والأسر المحددين لوضع وتنفيذ خطة عمل تتوافق مع احتياجاتهم وقدراتهم وأهدافهم.
• مناقشة الخيارات المتاحة مع العملاء والأسر، بما في ذلك البحث عن بدائل عند عدم توفر الخدمات المطلوبة، وشارك العملاء والأسر في اتخاذ القرارات التي تؤثر عليهم.
• تقييم المخاطر ودعم العملاء والأسر لفهم المخاطر المتعلقة بوضعهم.
• ربط العملاء والأسر مع مجموعات الدعم والشبكات والموارد المجتمعية الأخرى عند الاقتضاء.
• تحديد واستقبال الإحالات الخاصة بالعملاء والأسر ذوي الاحتياجات المحددة المكتشفة خلال التسجيل أو الأنشطة النفسية والاجتماعية الأخرى.
• توفير المعلومات للعملاء والأسر حول حقوقهم والامتيازات المتاحة لهم.
• ضمان المتابعة والمراجعة المناسبة للخدمات المقدمة للحالات.
• ضمان توثيق شامل ومحدث يتعلق بكل حالة فردية.
• تحديد ما إذا كان من المناسب إيقاف المتابعة للعملاء والأسر (إغلاق الحالة).
• تحمل المسؤولية أمام الأفراد والأسر والمجتمعات عن الخدمات المقدمة.
• تقديم الاستشارة النفسية والاجتماعية ودعم تنسيق تقديم الخدمات من قبل مقدمي خدمات متخصصين آخرين وفقًا لاحتياجات الأفراد والأسر.

إدارة البيانات والتقارير:

• إدارة وتخزين البيانات، مع ضمان سرية المعلومات المجمعة والخدمات المقدمة.
• إعداد وتقديم خطط العمل الأسبوعية والشهرية في الوقت المناسب لمراجعة ضابط إدارة الحالة.
• دعم تنفيذ أدوات الرصد والتقييم والإبلاغ عن المشكلات في تنفيذ البرنامج.
• المشاركة في الاجتماعات الأسبوعية للتخطيط والتقرير لضمان الالتزام بمعايير الرعاية وإدارة الحالات.

التنسيق والتمثيل:

• المساهمة في الحفاظ على خريطة خدمات محدثة لمقدمي الخدمات العاملين في المنطقة.
• استقبال الحالات المحالة من وكالات أخرى وتقديم التغذية الراجعة للأفعال المتخذة عند الضرورة ووفقًا لمتطلبات السرية.
• الضغط نيابةً عن الأفراد والأسر للوصول إلى الخدمات ودعمهم في تمثيل وجهات نظرهم واحتياجاتهم في جميع الاجتماعات التي تؤثر عليهم.

المؤهلات الأساسية:

• خبرة العمل: سنتان على الأقل من الخبرة في إدارة الحالات، ويفضل أن تكون الخبرة في العمل مع اللاجئين.

• المهارات والكفاءات:

  • القدرة على الحفاظ على السرية والاحترام وعدم التمييز وسلامة المستفيدين في جميع الأوقات.
  • مهارات تواصل جيدة.
  • مهارات التعامل مع الآخرين وحل المشكلات، والقدرة على الإبداع والمرونة.
  • روح العمل الجماعي، والقدرة على العمل في بيئة متعددة الثقافات.
  • القدرة على العمل لساعات إضافية عند الحاجة والتحمل في ظل الضغوط.

Job Description:

The Case Worker will be responsible for implementing Comprehensive Protection Case Management for individuals at heightened risk. The Case Worker will work under the supervision of the Protection Officer to ensure qualitative and effective case management.

Responsibilities:

Individual Case Management:

• Conduct household visits and center-based interviews to assess the needs of clients and families.
• Collaborate with identified clients and families to develop and implement action plans that align with their needs, capacities, and goals.
• Discuss choices and available options with clients, seeking alternatives when needed services are unavailable, and actively involve clients and/or families in decisions that affect them.
• Assess risks and support clients and families in understanding the risks relevant to their situation.
• Link clients and families to support groups, networks, and other community resources as appropriate.
• Identify and receive referrals for clients and families (including caregivers) with specific needs identified during registration, psychosocial outreach, and other activities (e.g., through Refugee Outreach Volunteers).
• Provide information to clients and families about their rights and available services.
• Ensure appropriate monitoring, review, and follow-up of services provided to cases.
• Maintain complete and updated documentation related to each individual case.
• Determine when it is appropriate to discontinue follow-up with clients and families (case closure).
• Be accountable to individuals, families, and communities for the services provided.
• Directly provide psychosocial counseling and support the coordination of services provided by other specialized service providers based on the needs of individuals and families.

Data Management and Reporting:

• Manage, file, and store data while ensuring the confidentiality of the information collected and services provided.
• Prepare and submit weekly and monthly work plans promptly for validation by the Case Management Officer.
• Support the implementation of monitoring and evaluation tools and report any issues in program implementation.
• Participate in weekly reporting and planning meetings to ensure adherence to care standards, quality case management, and reporting requirements.

Coordination and Representation:

• Contribute to maintaining an up-to-date service map of providers operating in the area and take proactive steps to remain informed about available services.
• Receive cases referred from other agencies (following case allocation by the Case Management Officer). Provide feedback on actions taken to other agencies when necessary and appropriate while respecting confidentiality requirements.
• Advocate on behalf of individuals and families to access services and support them in effectively representing their views, needs, and capacities in all meetings that affect them.

Essential Attributes:

• Work Experience: At least 2 years of experience in case management, with a preference for experience working with refugee populations.

• Demonstrated Skills and Competencies:

  • Ability to maintain confidentiality, respect, non-discrimination, and safety of beneficiaries at all times.
  • Good communication skills.
  • Excellent interpersonal and problem-solving skills, with creativity and flexibility.
  • Strong team spirit, comfortable in a multicultural environment.
  • Flexibility to work overtime when needed and ability to handle pressure well.
  • Proficient computer skills (Microsoft Word and Excel).
  • Strong reporting skills.

• Competency Profile:

  • Education: A degree in Social Work or a related field with a strong cash management component is preferred.
