Job Summary
As a Case Worker specializing in case management, they will play a crucial role in providing comprehensive support and guidance to individuals or families facing various challenges, any type of crisis or conflict, with no discrimination of any kind. Their responsibilities will include assessing individuals’ needs, developing integrated care plans, coordinating services across different agencies and providers, and advocating for beneficiaries to ensure they receive the necessary resources and support. They will work closely with a multidisciplinary team to address beneficiaries’ issues from multiple perspectives, promoting long-term stability and empowerment.
Main Tasks and Responsibilities
- Provide social support to beneficiaries through counseling, crisis intervention, and skills-building activities;
- Conduct through assessments the beneficiaries’ strengths, needs, and goals using a holistic approach;
- Develop individualized care plans that incorporate input from beneficiaries and families;
- Identify potential risks and protective factors to inform intervention strategies;
- Conduct home visits on a weekly basis;
- Monitor the beneficiaries’ progress and adjust intervention plans as needed;
- Intervene with varied thematic and vulnerabilities in the community;
- Plan a social intervention based on the case management integrated approach;
- Refer the beneficiaries to the appropriate services when needed;
- Ensure a proper registration and documentation of the new cases received through the hotline or in-person;
- Coordinate with the CM program coordinator regularly;
- Ensure a proper documentation and reporting respecting the deadlines and standards;
- Prepare weekly and monthly plans and send them to the CM program coordinator;
- Attend regularly the internal sessions with other respective programs;
- Identify vulnerable cases in need of individual specialized intervention and provide them with structured social intervention;
- Participate to workshops, trainings, team activities.

Qualifications and Experience
- Bachelor’s degree in social work or in psychology is a must (any other major will not be considered);
- Master’s degree is a plus;
- Proven experience in case management, preferably with a focus on integrated service delivery;
- Knowledge of community resources, social services, and relevant ethical standards;
- Strong communication, interpersonal, and organizational skills;
- Ability to work effectively both independently and as part of a team;
- Cultural competence and sensitivity to diverse populations and backgrounds;
- Preferably familiar with PFA guidelines and have attended various workshops and trainings focused on field work, communication skills and behavioral skills;
- Proficiency in Microsoft office (Word, Excel).

Conditions
- Full-Time working hours: 5 days per week, 8 hours per day, from 9:00 am till 5:00 pm.
- Willingness to travel abroad for meetings and conferences (where applicable).
